Travel Nurse RN Home Health Case Management Opportunity

Job Summary

We are looking for a highly motivated and experienced Travel Nurse RN Home Health Case Manager to provide high-quality patient care in a home health setting. The successful candidate will have a strong background in case management and be able to work independently in a fast-paced environment.

Responsibilities
  • Provide comprehensive case management services to patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Conduct regular assessments and evaluations to ensure patient safety and well-being
  • Document all patient interactions and care activities in a timely and accurate manner
Requirements

To be considered for this opportunity, you must have:

  • A current RN license in the state of Illinois
  • At least 2 years of experience in home health case management
  • Strong knowledge of case management principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
