We are seeking a highly motivated and detail-oriented Registered Nurse to fill the position of Renal Care Coordinator at Northwell Health. About You:

Responsibilities:

  • Manage patient referrals and coordinate care transitions between departments.
  • Develop and implement individualized treatment plans in collaboration with the healthcare team.
  • Communicate with patients, families, and healthcare staff to ensure informed decision-making and optimize care delivery.
  • Maintain accurate and up-to-date patient records, including treatment plans, test results, and medication lists.
  • Participate in quality improvement initiatives and contribute to the development of policies and guidelines.

Qualifications:

  • Graduate from an accredited School of Nursing (BSN preferred).
  • Current License to practice as a Registered Professional Nurse in New York State required.
  • Specialized certifications (e.g., CRRT, Nephrology) desired but not required.

Estimated Salary:** $80,000 - $110,000 per year (based on location and experience).
