**Job Overview**
">We are seeking a highly skilled Primary Care Physician Specialist to join our team at Conviva Care Centers. As a key member of our healthcare team, you will be responsible for providing proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
**Responsibilities**
">- Evaluate and treat center patients in accordance with standards of care.
- Follow level of medical care and quality for patients and monitor care using available data and chart reviews.
- Assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
- Act as an active participant and key source of medical expertise with the care team through daily huddles.
- Help Regional Medical Director and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
**Requirements**
">- Current and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Graduate of accredited MD or DO program of accredited university.
- Excellent verbal and written communication skills.
- Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
- Fully engaged in the concept of "Integrated team-based care" model.
**Compensation and Benefits**
">The estimated salary for this position is **$199,400 - $298,300 per year**, depending on experience and qualifications. Our comprehensive benefits package includes medical, dental, and vision benefits, 401(k) retirement savings plan, time off (including paid time off, company and personal holidays, volunteer time off, paid parental and caregiver leave), short-term and long-term disability, life insurance, and many other opportunities.
