Hospice Case Manager RN

Job Description
We are seeking a highly skilled and empathetic RN Case Manager to join our team at Heart of Hospice in Hattiesburg, MS. As a key member of our interdisciplinary group, you will be responsible for providing comprehensive care to patients and their families.
Key Responsibilities:
- Conduct initial nursing evaluations to determine eligibility for hospice services.
- Develop and implement personalized patient plans of care.
- Collaborate with patients, families, and healthcare professionals to ensure seamless care delivery.
- Document patient progress and outcomes accurately and timely.
Requirements:
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
