This is a home-based primary care opportunity with a high population of geriatric patients and limited access to geriatric-specific care. The ideal candidate will have experience in geriatric care and be comfortable working in a clinic and home visit setting.

Responsibilities
  • Provide primary care services to geriatric patients in a clinic and home visit setting
  • Manage patient care and make decisions regarding treatment and referrals
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
Requirements
  • Board Certified in Family Medicine or Internal Medicine
  • Active CA License or IMLC
  • Experience in geriatric care and comfortable working with geriatric patients
  • Ability to work in a clinic and home visit setting
