We are seeking an experienced and team-oriented RN Triage Case Manager to join our team at Tidewell Hospice.
About the RoleThe RN Case Manager will create and maintain a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient's hospice experience.
Key Responsibilities- Review and update the care plan, physician orders, pharmacy profile, and other records.
- Educate patients, family members, and other caregivers about the disease process and decline, available palliative interventions, the dying process, and safety practices.
Applicants must be a graduate of an approved nursing program and own a current Florida RN license. BSN or MSN preferred. Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
