We are seeking a highly skilled and compassionate Urology Nurse Navigator to join our team at Trinity Health Corporation. As a key member of our healthcare team, you will play a vital role in ensuring seamless care for patients undergoing urology procedures.
Key Responsibilities- Coordinate patient care to streamline comprehensive surgical care for each patient
- Communicate effectively with colleagues, patients, and family members to ensure clear understanding of treatment plans and progress
- Provide education and support to patients and family members throughout the diagnosis and treatment process
- Collaborate with radiology, pathology, laboratory, and primary care services to ensure seamless care for patients
- Develop and manage care plans to meet individual patient needs
- Graduate of an accredited School of Nursing with a Bachelor of Science in Nursing (BSN) degree
- Current licensure as a registered nurse (RN) in Connecticut
- At least 3 years of practical experience in surgical nursing and familiarity with new trends and practices in surgery
- BLS certification required
- National certification in area of specialty preferred
