At Interim HealthCare, we're committed to providing exceptional care to our patients and their families. As a Hospice Intake Nurse, you'll play a vital role in ensuring that our patients receive the best possible care and support.
Key Responsibilities:- Act as a liaison between hospice referral sources, outside agencies, and health professionals to coordinate patient care.
- Communicate with patients and families to arrange for the initiation of care services.
- Complete intake screenings, including obtaining and documenting required information to make preliminary admission decisions.
- Prepare reports and responses to inquiries, and provide telephonic contact as needed.
- Actively promote care services to prospective patients and families.
- Graduate of an accredited school of nursing.
- Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Michigan.
- One year of experience as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
- Prior hospice or home health care experience preferred.
