Job Summary

The Cardiac Health Navigator RN is a key member of the Phoebe Physicians team, responsible for coordinating individualized care for patients diagnosed with ongoing critical cardiac health conditions. As an advocate, educator, consultant, leader, and community ambassador, this role serves as a consistent care coordinator throughout the continuum of care, assessing the physical, psychological, and social needs of patients.

Key Responsibilities
  • Guide patients and families through the complex healthcare system, eliminating barriers that may impact patient care and outcomes.
  • Coordinate the efforts of the medical team to facilitate continuity of care and enhance patient satisfaction.
  • Stay up-to-date on new and changing regulatory requirements, hospital policies, and clinical procedures to ensure compliance and quality care.
  • Provide post-discharge support to patients via phone calls and systematic visits, ensuring they have the necessary knowledge and resources to manage their chronic cardiac disease.
Requirements
  • 2 years of nursing experience and experience in Cardiology.
  • Registered Nurse (RN) with current Georgia license and Basic Life Support (BLS) certification.
  • Nurse Navigator Certification in Congestive Heart Failure within 12 months of employment.

  • Ability to work in a fast-paced environment and prioritize tasks.
  • Basic computer skills and proficiency in Microsoft Office Suite.
Working Conditions
  • Works in a well-lighted, air-conditioned area with moderate noise levels.
  • May be exposed to high noise levels and bright lights.
  • May be required to change from one task to another of different nature without loss of efficiency or composure.
