Nurse Practitioner

As a Registered Nurse with Hospice of Michigan, you will provide compassionate and high-quality care to patients and their families in their time of need. This role requires a strong commitment to delivering exceptional patient-centered care, working collaboratively with the interdisciplinary team, and promoting a culture of empathy and understanding.
Key Responsibilities- Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop individualized plans of care to address these needs.
- Provide end-of-life care, including symptom management and comfort care, in accordance with NorthStar Care Community indicators.
- Collaborate with the interdisciplinary team to prioritize and provide patient and family-oriented hospice services, ensuring seamless communication and coordination of care.
- Supervise and instruct Hospice Aides in providing nursing care to patients, adhering to regulatory guidelines.
- Participate in quality assurance processes, internal and external education and training, marketing, and other activities that promote the mission of Hospice of Michigan.
- Graduation from an accredited school of nursing with a current license to practice as a registered professional nurse in the State of Michigan.
- Minimum of one to two years of varied nursing experience, with hospice experience and CHPN certification preferred.
- Ability to use technology, including hardware and software, required of clinicians in their practice.
- Excellent interpersonal skills, sound judgment, effective organizational and prioritization skills, and attention to detail.
