Job Title: RN Onsite Assessor

Join CHCS Services, Inc as a RN Onsite Assessor and play a vital role in assessing the eligibility of clients for Long Term Care Insurance. As a Registered Nurse, you will be responsible for conducting non-medical, observational assessments in the client's home or facility setting.

Key Responsibilities:

  • Conduct cognitive and functional assessments to determine the client's level of independence
  • Document client demographics, physician information, and medication and diagnosis lists
  • Administer cognitive screenings (MMSE or SPMSQ) and document the client's ability to complete ADL and IADL tasks
  • Submit completed assessment documentation within 24 hours of the scheduled appointment
  • Remain available for clarifications and answer any questions from the Clinical Review team

Requirements:

  • Current unencumbered RN licensure in the state of practice
  • Must have or be willing to obtain a professional liability insurance policy
  • Must have a current driver's license, vehicle insurance, and access to dependable vehicle and/or public transportation
  • Minimum of 2 years of home health experience or nursing in an equivalent area of practice serving geriatric clients
  • Must have your own computer/tablet and internet access
