- Evaluate patients in their homes or hospital settings and coordinate care within the Chapters Health System network.
- Facilitate a seamless transition to Care Navigation and represent the organization effectively.
- Bachelor's degree in nursing or a related discipline, or a comparable combination of education and experience.
- Valid Florida RN license or equivalent certification.
- At least 2 years of RN experience in acute care, hospice, home health, or post-acute care environments.
- Strong clinical assessment capabilities, preferably with experience in emergency departments or intensive care units.
- Familiarity with EMR/EHR systems is essential.
- Proficient English communication skills; Spanish language skills are a plus.
- Ability to present services in a professional and clear manner.
- Possession of a valid driver's license and dependable transportation.
- Team-oriented individual capable of managing multiple priorities and high-stress situations.
- Excellent time management skills and a focus on customer service.
- Current BLS certification from AHA or Red Cross.
- Conduct clinical admissions for all services by clearly explaining offerings to patients and families, addressing their concerns, and educating them on admission processes.
- Collaborate with healthcare providers to initiate the Plan of Care.
- Deliver skilled nursing interventions and secure necessary orders.
- Serve as the organization's representative at designated facilities, facilitate referrals, and communicate effectively with Care Navigation personnel.
- Obtain necessary consents for services and ensure safe discharge for eligible patients.
- Build strong relationships with case managers, physicians, and other key stakeholders.
- Provide care for GIP patients in hospital settings and manage daily visits and discharges efficiently.
- Perform additional duties as assigned.
