OBJECTIVE AND SCOPE:
Contributes to the mission, vision, core values, and customer service philosophy of Fresenius Medical Care North America. Complies with the FMCNA Compliance Program, adhering to all regulatory and policy requirements.
Acts as a vital member of the hemodialysis healthcare team, delivering safe and effective dialysis treatment for patients under the direct oversight of a licensed nurse, in accordance with FMCNA policies, procedures, and training, while complying with regulations established by corporate, state, and federal agencies.
Promotes the FMCNA commitment to the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) initiatives, focusing on patient satisfaction and actively engages in process improvement activities to enhance the likelihood of achieving FMC Quality Enhancement Goals (QEP).
RESPONSIBILITIES / TASKS:
CUSTOMER SERVICE:
- Upholds the FMS culture through values and customer service standards.
- Ensures exceptional customer service to all external and internal stakeholders.
- Cultivates and maintains effective relationships through timely and effective communication.
- Takes proactive measures to address, resolve, and follow up on customer service issues promptly.
PATIENT CARE:
- Assists healthcare team members in providing patient-specific education regarding adequacy measures where applicable.
- Welcomes assigned patients and checks on their wellbeing since their last treatment, reporting any concerns to the nurse supervisor.
- Evaluates vascular access for patency and communicates any unusual findings to the nurse supervisor.
- Obtains necessary pre and post-treatment vital signs and weight, reporting any irregularities to the nurse supervisor.
- Conducts vascular access evaluations pre-treatment and reports any unusual findings to the nurse supervisor.
- Performs vascular access cannulation and communicates any difficulties or unusual findings to the nurse supervisor.
- Administers Heparin as delegated or permitted by state law.
- Initiates dialysis treatment according to prescribed orders, including blood flow and dialysate flow.
- Assesses intradialytic issues and provides interventions as prescribed by physician orders or as directed.
- Monitors patients' responses to dialysis therapy and reports any unusual findings to the nurse supervisor.
- Obtains vital signs prior to reinfusion and performs all necessary functions for the discontinuation of treatment, documenting appropriately.
- Discontinues dialysis treatment following established procedures.
- Evaluates the patient prior to termination of venous access, including standing and sitting blood pressure assessments.
- Achieves hemostasis and applies appropriate dressings.
- Evaluates the patient prior to discharge and reports any unusual findings to the nurse supervisor.
- Performs and records pre and post-dialysis evaluations, weight, and vital signs with initial identification.
- Responsible for calculating and entering individual patients' dialysis machine programming for Ultrafiltration goals, treatment time, and sodium modeling as prescribed.
- Ensures that all safety alarms are enabled, including Narrow Venous Limits.
- Records accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
Reports any significant information and/or changes in patient condition directly to the nurse supervisor.
Observes patients and conducts machine safety checks according to facility policy, reporting any changes or unusual findings to the nurse supervisor.
Documents any interventions for unusual patient status and the patients' responses to those interventions.
Evaluates patients' vascular access during treatment, including arterial and venous monitoring pressures, providing appropriate interventions as needed, documenting, and reporting any unusual findings to the nurse supervisor.
Obtains all prescribed laboratory testing and prepares specimens for collection.
MAINTENANCE/TECHNICAL:
Maintains environmental integrity and aesthetics, ensuring all areas are safe and clean.
Cleans and disinfects dialysis machine surfaces, chairs, equipment, and surrounding areas between treatments according to facility policy.
Conducts all necessary tasks for preparation for dialysis treatment and documents where appropriate.
Performs all required pretreatment dialysis machine alarm testing.
Initiates the Solution Delivery System.
Ensures that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
Prepares, organizes, and efficiently utilizes supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
Completes and documents ongoing participation in Continuous Quality Improvement activities.
Enters all treatment data into the designated clinical application accurately and in a timely manner.
Reviews treatment sheets for completeness, ensuring nursing signatures are documented, and ensures omitted entries are completed or corrected where appropriate.
Prepares initial patient charts for admission and compiles paperwork for appropriate placement in charts.
Prepares lab requisitions for prescribed or stat lab specimens according to laboratory destination.
Collects, labels, appropriately prepares, and stores lab samples according to required laboratory specifications.
Ensures collection of lab specimens by the appropriate lab courier.
OTHER:
Performs additional duties as assigned.
EDUCATION:
Center for Medicaid/Medicare Services (CMS)-approved state and/or national certification or High school diploma or G.E.D. and must meet certification requirements within the required state or CMS timeline.
All appropriate state licensure, education, and training (if any) required.
EXPERIENCE:
Previous patient care experience in a hospital setting or related facility preferred.
Employees must meet the necessary requirements of Ishihara's Color Blindness test as a condition of employment.
Continued employment is contingent upon successful completion of the FMCNA dialysis training program and CPR certification.
