We are seeking experienced Advanced Practice Providers to join our in-home care team, where you will work alongside a clinical care team to provide comprehensive health assessments, diagnosis, treatment, and outcome management for patients of all ages.
Key Responsibilities:- Deliver care in the patient's home through innovative in-home care service lines
- Perform therapeutic interventions, educate patients and families, and effectively communicate with all individuals involved in the patient's care
- Collaborate with the clinical care team to provide comprehensive care
- Minimum of two years of experience as an APP in relevant areas
- Graduate with an advanced degree from an accredited institution
- Current unrestricted state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified, current
- Current BLS required, ACLS certification preferred
- Prescriptive authority and DEA
- Valid driver's license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
