Our Palliative Care Registered Nurse will directly report to the Director of Palliative Care.
In this role, you will collaborate with patients, their families, and a multidisciplinary healthcare team to deliver and coordinate high-quality end-of-life care.
Palliative Care RNs conduct both in-person and telephonic evaluations for patients, working closely with the interdisciplinary team.Your primary responsibilities will include conducting in-home assessments in private residences, assisted living facilities, and skilled nursing homes. You will also provide education to patients and their families, ensuring a comfortable home environment.
This position is a weekday role, operating Monday through Friday, from 8 AM to 5 PM. *Experience in hospice or palliative care is required.- As a Registered Nurse, your responsibilities will include: Conducting in-person and telephonic assessments, and providing education to palliative care patients, their families, or caregivers as necessary.
Performing Risk Stratification Assessments to determine patient support needs in accordance with established protocols, in collaboration with the Nurse Practitioner. Monitoring patient visit frequency based on individual needs and risk assessments. Delivering reliable education and guidance to patients and families, ensuring excellent customer service that brings comfort and reassurance. Consulting with and educating patients and families about disease processes, self-care techniques, end-of-life care, medications, nutrition, and dietary requirements. Communicating findings that indicate necessary preventive and rehabilitative skilled nursing or therapy needs to the Primary Care Provider or Palliative Nurse Practitioner. Educating patients and families on medication administration and treatments as outlined in the care plan. Coordinating care with members of the palliative team. Reporting any changes in patient condition or signs of decline to the Palliative Nurse Practitioner and/or Primary Care Provider. Supporting hospice referrals as appropriate. Documenting clinical records in a timely manner, including screening and assessment findings, physician orders, progress notes, and care plans. Assisting in the development and implementation of palliative care plans from initial assessment through discharge.
Assessment Responsibilities:
Your assessments should encompass:
changes related to the disease, functional decline, nutritional status, cognitive changes, inadequately managed pain or distressing symptoms, falls, infections, or similar incidents, and the overall impact of the disease on the patient's condition and the quality of life for caregivers and family members.
Collaborate on the creation and adjustments of the palliative care plan tailored to each patient, in consultation with the patient, family, and other members of the Palliative Care Team.
Facilitate the provision of care when needs are identified in line with the Plan of Care.
Assist in transmitting prescriptions to pharmacies, including controlled substances as permitted by state law. Ensure timely communication of laboratory results and other diagnostic tests to the Nurse Practitioner. Request necessary durable medical equipment as indicated.Communicate clinical findings to the Nurse Practitioner and/or Primary Care Provider as needed, at least bi-weekly. Regularly reassess the effectiveness of the care plan and make modifications as necessary.
Maintain effective working relationships with the supportive care team (e.g., Social Worker) as applicable, involving them when the patient's care plan requires team participation to achieve desired outcomes.
Establish and nurture effective working relationships with all contacts during the course of your work.Document clinical findings, treatment plans, and care provided in the patient's medical record in accordance with accepted standards to support sound nursing practice.
Candidate Profile:

Registered Nurse Education:
Minimum of 2 years of prior experience as a skilled Registered Nurse. Proven ability to deliver quality patient care. Preferred experience in palliative care, oncology, hospice, or geriatric care. Additional experience that may contribute to success in this role includes: hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing. Licenses, Certifications, and/or Registration: Current RN license to practice in the state. CPR Certification. Proficiency in Microsoft Outlook, Word, and Excel. Competence in electronic medical record documentation. Specialized Knowledge and Skills: Previous clinical experience. Strong compassionate communication skills. Ability to make independent, sound decisions regarding patient care. Competent in performing after-hours in-person visits. Familiarity with the needs of patients and families facing advanced illnesses. Excellent written and verbal communication skills. Commitment to maintaining patient confidentiality. Strong problem-solving and time management skills.
