Transplant Coordinator

We are seeking a highly skilled and compassionate Transplant Coordinator to join our team at Ochsner Health System. As a key member of our transplant team, you will play a vital role in ensuring the smooth transition of patients through the transplant process.
Responsibilities:
- Performs pre/post transplant clinical activities, including monitoring patients and managing outpatient clinics.
- Educates patients on procedures and provides support to ensure their independence and self-care.
- Acts as a patient advocate and educator, promoting optimum patient rehabilitation and outcomes.
- Collaborates with transplant physicians to ensure seamless patient care and management.
- Participates in quality initiatives and ensures compliance with regulatory requirements.
Requirements:
- Registered Nurse Diploma or Associate Degree in Nursing.
- 3 years of related experience with RN Diploma or RN Associate Degree or 2 years of related experience with a BS Degree in Nursing.
- Current RN License in the state of practice.
- Preferred: Certified Clinical Transplant Coordinator (CCTC) through the American Board of Transplant Certification (ABTC).
