Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As a Homecare LPN, you will be responsible for assessing, planning, implementing, and evaluating patient care to ensure optimal health outcomes.

Key Responsibilities:
  • Utilize the nursing process to assess patient needs and develop individualized care plans.
  • Provide skilled nursing care and services in accordance with prescribed orders and medical protocols.
  • Collect and analyze data to identify potential health risks and develop strategies to mitigate them.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
Requirements:
  • Current licensure as an LPN/LVN in the state of practice.
  • Current TB or Chest X-Ray clearance.
  • Current BLS certification.
