Key Responsibilities:

Accountable for delivering internal medicine services to patients. Core responsibilities may include, but are not limited to:

  1. Developing a thorough and coordinated care plan across various environments including hospitals, outpatient clinics, and Skilled Nursing Facilities (SNFs), considering medical records, patient and family input, primary care providers, and specialists.
  2. Conducting daily hospital rounds.
  3. Collaborating with hospitals and care management teams, reviewing patient cases with case managers.
  4. Providing care in the Care Center (CCC) and supporting nurse practitioners in case evaluations.
  5. Caring for patients in SNFs and participating in SNF meetings to discuss patient cases.
  6. Available for on-call duties during nights and weekends as scheduled.
Qualifications:

Must possess an MD; completion of an Internal Medicine residency; a minimum of 2 years of experience as a Hospitalist. Requires current unrestricted medical licensure in relevant state(s) and board eligibility or certification. DEA license is mandatory.
