Key Responsibilities:
  • Evaluate and assess new patients to develop effective care plans.
  • Create and update comprehensive short and long-term patient care plans to ensure optimal outcomes.
  • Educate families and patients about available resources and healthcare options to empower informed decision-making.
  • Collaborate with other RNs, doctors, and social workers to provide seamless care coordination.
  • Properly document and submit medical records to maintain accurate patient information.
  • Communicate with insurance companies to collaborate or appeal decisions, ensuring patients receive necessary care.
  • Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial status.

Requirements:
  • BLS certification
  • 2 years of recent case management experience

Additional Information:
As a Case Management Travel Nurse, you should be able to stand and walk for long periods, carry up to 10 pounds of weight, and bend, lean, and stoop without difficulty. You'll work efficiently in a collaborative environment, managing a fluctuating caseload during times of high census. Good oral and written communication skills are essential to communicate effectively with patients, families, and co-workers.
