Key Responsibilities- Coordinates patient care plans, including assessment, intervention, and monitoring.
- Develops and implements effective care plans to address patient needs and barriers to disease management or recovery.
- Coordinates transition of care to post-acute services, including arranging for durable medical equipment and home health services.
- Documents patient assessment findings and care plan interventions in the clinical record.
- Communicates effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
- Associate's or Bachelor's degree in Nursing, Social Work, or Counseling.
- 3 years of experience in nursing, social work, or counseling.
- Registered Nurse (RN) licensure and/or Compact State Licensure.
- Certified as a Board Certified Case Manager within 1 year of accepting position.
- Cardiopulmonary Resuscitation (CPR) certification.
- Master's degree in Nursing, Social Work, or Counseling.
- Licensed Medical Social Worker (LMSW) or Licensed Professional Counselor (LPC) licensure.
