Job Summary and Qualifications
The Case Coordinator (CC) ensures high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes.
Key Responsibilities:
- Provide program orientation to patients, families, and caregivers, covering the case manager's role, rehabilitation philosophy, and criteria for continued stay and discharge. This includes information on Medicare and insurance benefits, grievance procedures, treatment planning, and rights and responsibilities.
- Act as the coordinator for patient, family, and caregiver education, encouraging their involvement in team discussions regarding plans, goals, and status through Family Conferences and other interactions.
- Ensure the implementation of the patient's treatment plan that aligns with their strengths, abilities, needs, and preferences, facilitating patient involvement throughout the rehabilitation process.
- Document findings from the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating the social, financial, or discharge needs and preferences of the patient, family, and caregivers.
- Promote consistent, positive patient interactions that enhance the agenda of exceptional patient service.
