**Per Diem Infusion Nurse

We are seeking a highly skilled and compassionate Per Diem Infusion Nurse to join our team at Soleo Health Inc. As a Per Diem Infusion Nurse, you will play a critical role in providing primary patient care to patients receiving infusion therapy and other nursing services in the home or alternate site setting.
Key Responsibilities- Provide Direct Patient Care: Assess, plan, implement, and evaluate patient care to ensure safe and effective infusion therapy.
- Communicate Effectively: Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
- Assess and Educate: Evaluate patient/care partner education needs and provide instruction on safe administration of home infusion therapy.
- Coordinate Care: Develop and maintain caseload scheduling, coordinate care with other team members, and ensure timely submission of plan of treatments.
- Participate in On-Call Rotation: Provide continuous nursing support services to patients outside of regular business hours.
- RN Degree: Bachelor of Science in Nursing (BSN) preferred.
- Experience: Minimum 3 years of nursing experience with background in infusion therapy preferred.
- Licensure: Current nursing license for applicable home state.
- Skills: Proficient with Microsoft Outlook and Microsoft Office Suite, valid state-issued driver's license with no significant restrictions.
