Job Title: Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Interim HealthCare of the Black Hills. As a Physical Therapist, you will be responsible for providing high-quality patient care and rehabilitation services to clients in their homes.

Job Summary:

The Physical Therapist will work closely with our Home Healthcare Team to develop and implement individualized therapy plans that meet the unique needs of each client. This is a rewarding opportunity to make a positive impact on the lives of others and work in a dynamic and supportive environment.

Responsibilities:
  • Conduct thorough assessments of clients to identify their physical therapy needs
  • Develop and implement individualized therapy plans to meet the unique needs of each client
  • Provide high-quality patient care and rehabilitation services to clients in their homes
  • Collaborate with our Home Healthcare Team to ensure seamless care coordination
  • Document therapy goals and services provided
Requirements:
  • Licensed Physical Therapist
  • CPR Certified
  • Compassionate and friendly
  • Adaptable and a creative problem-solver
