Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-management of chronic illnesses.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Participate in ongoing education and training to stay up-to-date on best practices and evidence-based care.
- 1 year of experience as a RN in acute care, rehab, or homecare.
- Valid NYS RN and DMV licensure.
- Ability to work independently and as part of a team.
- Competitive pay rate of $49.95 per hour.
- Flexible scheduling with minimal weekend and holiday rotations.
