Job DescriptionWe are seeking a highly skilled Mental Health Specialist to join our team. As a key member of our multidisciplinary primary care treatment team, you will work closely with patients, medical staff, and community resources to provide comprehensive behavioral health services.
Main Responsibilities:
- Provide behavioral health screening, brief intervention, education, and referral services for patients presenting in our primary care clinics.
- Collaborate with primary care providers to develop and implement personalized treatment plans for patients with psychosocial, behavioral health, chronic disease, and substance use concerns.
- Engage in ongoing assessment and quality improvement strategies to enhance the integration of primary care and behavioral health services.
- Provide consultative services to healthcare team members and training to other professionals as needed.
- Maintain accurate documentation and adhere to all organizational policies and professional practice standards.
Qualifications:
- Master's Degree in a related field (e.g., social work, counseling, psychology).
- Licensed Clinical Social Worker Certification (LCSW or LCSW-A) in good standing with the Social Work Certification and Licensure Board of North Carolina (or equivalent).
- Minimum two years' experience in a public health setting.
