In this role, you will play a vital part in providing high-quality care to patients and their families, ensuring they receive the support and services they need to live their lives to the fullest.
Key Responsibilities:
- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
- Administer medication and treatment as prescribed by the physician, while also providing emotional and spiritual support to patients and their families.
- Ensure compliance with all state and federal legal and regulatory requirements, while maintaining accurate and up-to-date clinical notes.
Requirements:
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
