Responsibilities

Key Responsibilities:

  • Provide medication management services in coordination with healthcare professionals to ensure safe, effective, and cost-efficient medication therapy for patients.
  • Offer expert-level consultation, guidance, and direction for appropriate hematology and oncology-affiliated medication use.
  • Optimize cancer patient outcomes by providing evidence-based, patient-centered drug therapies and monitoring for drug-related problems and medication cost containment.
  • Support regulatory requirements and local quality programs related to safe medication use and promote medication stewardship and surveillance.
  • Detect, monitor, document, and report adverse drug reactions and medication errors.
  • Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
  • Communicate ongoing patient information and efficacy and safety of treatment regimen(s) to all care providers.
  • Lead and participate in interdisciplinary hospital committees, protocol development, and education of pharmacists, nurses, physicians, and other providers.
  • Coordinate current patient issues and therapy recommendations with pharmacy staff.
  • Support research activities of the respective specialty team as requested.
  • Stay up-to-date with changing protocols and clinical pharmacy standards as they pertain to applicable areas of specialty (e.g., oncology).
  • Participate in quality improvement, research activities, and education as described in the job-specific competencies.
Qualifications

Qualifications:

  • Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.
  • Preferred Qualifications:
    • Completion of a Post-Year Graduate 2 (PGY2) residency in a specialty practice area AND board certification in area of specialty obtained within 12 months of hire; OR, Post-Year Graduate 1 (PGY1) residency in a specialty practice area AND a minimum of two (2) years' post-pharmacist licensure experience within the last seven years with at least 50% of time spent in specialty practice activities as defined by BPS AND current, active board certification in area of specialty; OR, If no residency training, then a minimum of four (4) years' post-pharmacist licensure experience within the last seven years with at least 50% of the time spent in specialty practice activities as defined by BPS AND current, active board certification in area of specialty.
    • Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations.
    • Experience and/or interest in teaching and/or precepting. The candidate is expected to become a PGY2 Oncology Resident preceptor and meet the requirements set forth by ASHP.
    • Commitment to excellence, accuracy, attention to detail, and teamwork.
    • Ability to communicate effectively, work collaboratively with others, organize time well, practice with an agile mindset, solve problems, and work independently with minimal supervision.

License or Certification: Current pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site's applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
