We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Job Summary

The LPN/LVN will be responsible for delivering and coordinating quality patient care in compliance with physician orders. This includes observing and assessing patient condition and care needs, documenting all activities and assessments, and participating in the development and update of the nursing care plan.

Responsibilities
  • Implement and update the nursing care plan to ensure the best possible outcomes for patients.
  • Document all activities, assessments, and nursing actions in a timely manner.
  • Participate in the development and update of the nursing care plan.
  • Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk.
  • Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care utilizing infection control measures that protect both the staff and the patient according to OSHA standards.
  • Educate patients and families regarding the disease process, self-care techniques, and prevention strategies.
Requirements
  • Graduate of an accredited school of nursing.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • One year of pediatrics experience in South Carolina.
  • One year of experience required working under current nursing license in California.
  • Six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
