Job Overview
The Role:
Saint Luke's Health System is in search of a dedicated Case Management Assistant to contribute to our mission of providing exceptional care. This role is integral to the treatment team, collaborating with Behavioral Health Technicians, Registered Nurses, Individual Therapists, and Medical Providers. Your skills will be essential in delivering trauma-informed support to the youth in our Psychiatric Residential Treatment Facility (PRTF) program.
Work Schedule:
Monday – Friday, 8:30 AM to 5:00 PM
Key Responsibilities:
- Schedule follow-up appointments for residents post-discharge
- Participate in weekly treatment team meetings
- Coordinate monthly treatment reviews
- Manage case management tasks for residents, including clothing vouchers, visitation arrangements, and school enrollment
- Facilitate referrals and admissions to the PRTF program
We are looking for a proficient case manager with experience in child and adolescent care. The ideal candidate will possess a solid understanding of Trauma-Informed Care principles. This position demands strong organizational skills to effectively manage daily workloads and prioritize tasks. Excellent verbal and written communication skills, along with a compassionate approach, are essential. Success in this role requires collaboration, relationship-building abilities, and adaptability to changing priorities. You will be expected to work both independently and as part of a team.

A bachelor's degree in social work or a related field is preferred.
