Position Overview:
Aspirus Wausau Hospital is currently seeking a Registered Nurse - Acute Care Facilitator to enhance our healthcare team. This role involves evaluating patient requirements and orchestrating referrals to appropriate resources to fulfill those needs.
The Acute Care Facilitator employs nursing expertise to formulate the patient's care plan. As a vital member of the multidisciplinary team, this position collaborates closely with physicians, nursing staff, and ancillary departments, offering support and clinical insight regarding patient care coordination.
Key Responsibilities:
The Acute Care Facilitator is tasked with addressing social, economic, and environmental challenges that hinder patients from effectively managing their health needs. This role necessitates independent judgment, analytical thinking, problem-solving abilities, and collaborative skills to prioritize and organize multiple tasks while delivering patient-centered care.
Qualifications:
To be considered for this position, candidates should possess:
- An Associate's Degree in Nursing (BSN preferred).
- A minimum of one year of experience in an acute care hospital setting.
- Experience in critical care, general medical, and/or surgical nursing.
- Current RN licensure in Wisconsin or Michigan, depending on the assigned business unit.
