Job DescriptionJob Summary:
We are seeking a highly motivated and experienced Outreach Ambassador to join our team at BoldAge PACE. As an Outreach Ambassador, you will play a critical role in generating referrals into our PACE program and educating the community through community outreach, networking, and marketing activities.
Key Responsibilities:
- Develop and cultivate relationships with referral partners, healthcare providers, and senior service providers to promote our PACE program.
- Present at community events and conferences to educate the public about our PACE program and the benefits of our services.
- Collaborate with our interdisciplinary team to identify quality referrals and facilitate the assessment and eligibility determination processes.
- Develop and implement effective community outreach and engagement strategies to promote our PACE program and increase census growth.
- Represent BoldAge PACE at community events, conferences, and meetings to promote our services and build relationships with key stakeholders.
Requirements:
- Bachelor's degree in health care administration, business, communications, marketing, or a related human services field.
- Minimum 1 year of experience working with the elderly, preferably in a community outreach and engagement role.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with community members, referral sources, and co-workers.
- Knowledge of managed care environments, including current managed long-term care programs in the community and implications for PACE program growth.
- Ability to work independently and as part of a team, with a strong commitment to promoting quality services to a frail client population.
