About the Role:

We are seeking an experienced Hospice Registered Nurse (RN) to join our team in Burlington/Durham. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care, working closely with families, physicians, and other healthcare professionals to ensure comprehensive support.

Key Responsibilities:

  • Patient Care: Provide direct patient care, applying your knowledge of nursing theories and concepts to deliver effective and compassionate support.
  • Documentation: Maintain accurate and timely documentation of patient care, meeting reimbursement guidelines and agency standards.
  • Care Coordination: Work collaboratively with patients, families, physicians, and the Hospice team to coordinate physical care and develop personalized care plans.
  • Communication: Communicate effectively with patients, families, and agency personnel at all levels, ensuring seamless coordination and support.
  • Team Collaboration: Participate in IDT meetings and quality reviews, contributing to the development of high-quality care services.

Requirements:

To be successful in this role, you will need:

  • A current RN licensure in the state of practice.
  • A minimum of two years of clinical nursing experience, with at least one year of hospice nursing experience preferred.
  • A working knowledge of the principles and practice of nursing, including reimbursement guidelines and agency policies.
  • CPR certification and ability to commit to 12 hours of continuing education annually.
  • A willingness to accept travel assignments and weekend/evening/holiday call duties as needed.

About Liberty Hospice:

We offer a competitive salary of $83,000 per year, commensurate with experience. Our benefits package includes a $10,000 sign-on bonus, opportunities for professional growth and development, and a supportive work environment that values compassion, dignity, and strength.
