Job Summary:

Sweetser is seeking a skilled Clinician to join our team at Topsham Family Medicine. As a member of our care team, you will work collaboratively with physicians, advanced practice providers, and other practice staff to provide high-quality patient care.

Key Responsibilities:

• Actively participate in analyzing behavioral health screening tools and results.
• Demonstrate competence in clinical assessment of behavioral health conditions, including substance use disorders.
• Present facts, concerns, and options to patients and care team members in formal and informal methods.
• Implement clinical pathways and protocols for treatment of chronic and co-morbid conditions.
• Coordinate and manage referrals for patients with behavioral health conditions requiring longer-term intervention.

Requirements:

• Master's Degree in Social Work.
• Independently licensed to practice as an LCSW/LCPC in Maine (Conditional Considered).
• Valid State of Maine driver's license.
• Minimum of 2 years of experience in the social work field.
• Experience working with individuals of all ages, as well as with children and their families.
