Job Summary:The Licensed Practical Nurse will be responsible for providing skilled nursing services to patients in a variety of settings, including the patient's home, hospital, or long-term care facility. You will work collaboratively with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Assess the physical, psychosocial, and spiritual needs of patients and their families.
- Provide intermittent skilled nursing services to patients under the direction of their physician.
- Monitor and evaluate pain and symptoms, and provide information and assistance to patients and their families to manage those problems.
- Serve as a liaison to patients' physicians when needed, providing information and updates on patient care.
- Provide emotional support to patients and their families.
- Refer patients and/or family members to other services when appropriate and follow up as necessary.
- Provide health education and instruction to patients and their families.
- Maintain and complete all necessary plans and reports related to hospice care.
- Participate in care conferences and bereavement follow-up.
- Participate in case review, quality improvement activities, and other meetings as assigned.
- Licensed in the State of Ohio.
- Experience preferred in hospice care, home health, oncology, and/or pain and symptom management.
- Current CPR certification.
- Valid driver's license, good driving record, and auto accident insurance.
- Hospice and Palliative Care Certification desired.

- Compliance with Annual Flu Shot Policy or ability to provide exemption documentation.

- Ability to provide proof of eligibility to work in the United States.
- High level of integrity, ethics, and professionalism.
