McLeod Health seeks a skilled Professional Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care.

Estimated Salary: $65,000 - $85,000 per year

About the Role

The Social Worker II will be responsible for providing professional case management services to patients and families across various populations within the organization.

This includes:

  • Collaborating with interdisciplinary teams to develop and implement patient-specific care plans;
  • Maintaining professional relationships with patients, visitors, physicians, and co-workers;
  • Providing counseling services to patients and families when necessary;
  • Assisting with referrals to community-based resources;
  • Attending departmental in-services and staff meetings.

To succeed in this role, you will need strong communication and writing skills, as well as experience working in a social work agency or hospital setting.

A Master's Degree in Social Work is required, along with licensure as a Master's prepared Social Worker within the associated region of responsibility.
