Key Responsibilities:


  • Take and record vital signs, including temperature, pulse, and blood pressure.
  • Measure and record height, weight, and fluid intake/output to monitor residents' health status.
  • Recognize and report any abnormal changes in body functioning to a supervisor, ensuring prompt attention to residents' needs.
  • Assist with medication reminders, bathing, grooming, dressing, and other activities of daily living to promote residents' independence and dignity.
  • Transfer, position, and turn residents safely and efficiently to prevent falls and injuries.
  • Provide skin care and promote wound healing to prevent complications.
  • Contribute to the resident's assessment and plan of care, working collaboratively with the healthcare team to ensure comprehensive care.
  • Provide nursing rehabilitation and restorative nursing services to help residents regain strength and mobility.
  • Care for residents with dementia, using person-centered approaches to promote their well-being and quality of life.
  • Follow the schedule of residents' needs set out by the supervisor, prioritizing tasks to ensure timely completion.
  • Provide emotional and social support to residents, promoting their mental health and well-being.
  • Promote and protect residents' rights, respecting their autonomy and dignity at all times.
  • Inform the supervisor of any resident issues or concerns, ensuring prompt attention to their needs.
  • Respect and encourage residents' independence and dignity, promoting their self-esteem and confidence.
  • Respect residents' confidentiality, maintaining the trust and confidence placed in you.
  • Familiarize yourself with emergency equipment and procedures, ensuring your ability to respond promptly in case of an emergency.
  • Understand your duties during a fire drill and be familiar with the evacuation plan, ensuring your safety and the safety of others.
  • Attend all required training, in-service, and staff meetings, staying up-to-date on the latest best practices and policies.
  • Strive to maintain a safe working environment through the prevention of accidents, preservation of equipment, and achievement of safe working practices.
  • Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers, promoting a culture of respect and teamwork.

Requirements:


  • A High School Diploma or General Education Degree; or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Experience working in a skilled, sub-acute, assisted living, or memory care setting.
  • CNA licensure if required by state.
  • Basic computer skills.
  • Ability to work as part of a team and handle multiple tasks safely and effectively.
  • Proficient verbal and written communication skills.
  • Dedicated, compassionate, and energetic.
  • English fluency preferred.
