SECTION 1:
Job Overview

What is the Role of a Mohs Technician? The Mohs surgical technique is a precise method utilized for diagnosing and treating skin malignancies. As a Mohs technician, your role encompasses preparing surgical environments, assisting patients, and supporting the surgeon throughout the operation. This position is open to both trained and experienced Mohs Medical Assistants.

This role contributes to the diagnostic assessment of tissue through histological techniques. You will perform histological tasks in accordance with the protocols established by the histology and Mohs laboratory (whether in a clinic or a standalone facility).

SECTION 2:
Responsibilities and Duties

  • Execute a range of specialized histopathological techniques.
  • High Complexity Testing includes: Receiving fresh, unfixed specimens at the time of tissue extraction, mapping, grossing, sectioning of specimens, applying ink, performing relax cuts, and scoring tissue as necessary.
  • Non-High Complexity Testing includes: Embedding, cryotomy, staining, and coverslipping.
  • Conduct immunohistochemistry staining (in facilities that provide this service).


HIGH COMPLEXITY TESTING:


  • Specimen Collection/Triage: Accept surgical laboratory samples from patients for analysis. Confirm correct specimen orientation for mapping.

Verify patient mapping details:
Name, accession number, stage, tumor location, and diagnosis.

  • Specimen preparation: Ensure accurate specimen identification and orientation. Segment specimens into sections for processing as indicated on the map. Apply color-coded ink to specimens as specified on the map for margin orientation. Execute relax cuts and scores to tissue specimens as required. Position specimens on embedding slides for processing (confirm accession number/piece/stage).
NON-

HIGH COMPLEXITY TESTING:


  • Embedding: Correctly orient specimens on glass slides using the "reverse slide mount method" for embedding (typically with the epidermis to the left). Freeze specimens to glass slides, ensuring that the epidermis/base of the specimen are accurately placed for a complete representative section. Once frozen, invert specimens onto prepared object holder discs.
  • Microtomy (Cutting): Operate a microtome within a cryostat with appropriate settings, alignments, and knife angles to ensure optimal cutting/sectioning of specimens. Verify block orientation for accurate facing of specimens. Adequately trim specimen blocks to provide a true first margin of the specimen (excessive trimming may lead to false positives). Demonstrate the ability to produce acceptable quality representative specimen sections. Accurately place a series of sections on a microscopic slide (labeled with accession number/piece/stage). Correctly label and store cut specimen blocks within the cryostat chamber to ensure precise tissue identification.

    • Routine Staining Procedures: Select suitable stains and chemicals to perform H&E or TBlue staining procedures. Prepare and maintain stains and other solutions required for staining. Demonstrate the ability to identify acceptable and unacceptable staining. Recognize and rectify problem areas that lead to inadequate staining.
    • Mounting Procedure: Slides are hand cover slipped using a xylene-compatible mounting medium.
    • Specimens for Permanent Paraffin Processing: Prepare routine excisions to be processed as a "Slow Mohs" - non-frozen tissue. Thaw and submit previously frozen section (excision and biopsy) tissue for permanent paraffin processing. Prepare DAF (previously frozen) specimens for permanent paraffin processing.
    • Instrumentation: Maintain/service instruments according to established protocols. Demonstrate the ability to operate or utilize instruments for their intended purpose. Monitor and document instrument performance, recognizing malfunctions and identifying the need for repairs.
    • Reagent Preparation: Prepare reagents according to established procedures. Record quality control data for each reagent. Select appropriate storage and preservation methods for all reagents. Identify reagent malfunctions and the need for replacement.
    • General Safety: Demonstrate correct safety practices as defined in established protocols. Recognize and rectify hazardous situations related to job safety. Investigate and resolve complaints or refer unusual issues to a superior. Adhere to all OSHA regulations and universal precautions.
    • Chemical Hygiene/Biohazard Procedures: Demonstrate proper handling of chemicals and biohazard materials. Comply with all OSHA regulations and universal precautions, including waste and sharps disposal. Maintain knowledge of OSHA regulations and the use of universal precautions as appropriate.
      • General: Participate in department or unit Quality Improvement initiatives. Assist in general housekeeping duties for the Mohs Laboratory, including weekly cleaning. Maintain Mohs & DAF laboratory logbooks (accession/date/patient/tumor location).
      • Assist in evaluating new procedures and equipment for potential adoption in the laboratory environment.
      • Conduct quality control procedures on equipment, reagents, and products, maintaining proper records for quality control reports.
      • Prepare slides with permanent labels and file them appropriately.
      • Maintain Mohs/Excision logbooks.
      • Adhere to all safety policies and procedures while performing job duties and responsibilities, supporting a culture of high quality and excellent customer service.
      • Perform other duties as necessary or in the best interest of the organization.
