About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our hospice care team at Amedisys. As a Hospice Nurse, you will play a vital role in providing patient-centered care to individuals in need of end-of-life support.

Key Responsibilities
  • Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
  • Collaborate with the care team to ensure continuity of patient care and make referrals to other hospice care disciplines as needed.
  • Communicate patient progress to the clinical manager, physician, and care team, and document patient care progress records accurately and timely.
  • Educate families on patient condition, needs, and care, and provide emotional support as needed.
  • Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Requirements
  • Current RN license, specific to the state(s) you are assigned to work.
  • One+ year of nursing experience required.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
