JOB SUMMARY

The Personal Care Assistant plays a vital role as a supportive member of the home care team, operating under the guidance of a registered nurse or therapist. This position is essential in delivering various personal care services tailored to meet the unique needs of each patient. The aide is tasked with monitoring patients, documenting care provided, and reporting any significant observations.

KEY RESPONSIBILITIES

The responsibilities of the Personal Care Assistant encompass, but are not limited to, the following:

1. Delivering direct personal care, which includes:

a. Assisting with bathing

b. Providing back massages

c. Supporting oral hygiene

d. Administering shampoos

e. Changing bed linens

f. Aiding patients in dressing and undressing

g. Implementing skin care routines to prevent breakdown

h. Supporting toileting activities

i. Maintaining a clean and organized living space for the patient

2. Planning and preparing healthy meals.

3. Assisting with feeding when necessary.

4. Monitoring and recording vital signs, including temperature, pulse, respiration, and blood pressure, as directed (with verified skills competency).

5. Aiding in mobility and exercise as outlined in the care plan.

6. Performing range of motion exercises and other basic procedures as part of therapy services as instructed (with verified skills competency).

7. Executing simple procedures as an extension of nursing services as directed (with verified skills competency).

8. Assisting patients with self-administered medications.

9. Managing laundry tasks for the patient, as appropriate.

10. Ensuring patient safety and proper use of equipment (e.g., foot stools, side rails).

11. Reporting any changes in the patient's condition to the assigned nurse.

12. Complying with the organization's documentation standards and maintaining professional conduct.
