**Job Overview:** End of Life Care Nurse
We are seeking a dedicated and compassionate LPN to join our team as an End of Life Care Nurse in Ohio. This role requires a strong commitment to providing superior care and service to patients at the end of their life journey.
**Estimated Salary:** $60,000 - $70,000 per year
The End of Life Care Nurse will work collaboratively with the healthcare team to develop and implement individualized care plans for patients. The successful candidate will have excellent communication skills and be able to provide emotional support to patients and their families.
**Responsibilities:*
- Provide high-quality patient care at the bedside
- Collaborate with the healthcare team to develop and implement individualized care plans
- Document services and care provided to patients
- Communicate effectively with patients, families, and healthcare providers
**Requirements:*
- LPN license in the State of Ohio without any board actions
- One year of nursing experience preferred
- CPR certification
- Computer skills sufficient to properly document services and care
