Reports to: Clinical Manager

Position Summary: This role is chiefly focused on triaging patient care, encompassing case management, chronic care coordination, and acute assessments. The majority of duties will be conducted via telephone, with some in-person patient interactions. Opportunities for state and federal loan repayment are available for eligible candidates.

Key Responsibilities:

  • Deliver primary care nursing, patient education, and healthcare maintenance to clients at Peoples Community Health Clinic.
  • Utilize the nursing process (assessment, planning, implementation, and evaluation) to administer nursing care in collaboration with healthcare providers.

Protected Health Information Compliance:

  • This position necessitates the use or disclosure of protected health information, adhering to the Privacy Policies of Peoples Community Health Clinic, Inc.
  • Access to protected health information will be limited to treatment and healthcare operations.

Essential Duties:

  • Conduct comprehensive assessments of patients, including medical history and vital signs, following established protocols.
  • Maintain accurate patient records and document relevant information in the electronic health record (EHR).
  • Facilitate referrals and assist healthcare providers with procedures.
  • Administer medications, immunizations, and IV fluids as directed.
  • Provide patient education and counseling, reinforcing healthcare plans and offering emotional support.
  • Coordinate patient services to enhance workflow and efficiency.
  • Evaluate patient symptoms to determine necessary care and follow-up.
  • Engage in quality improvement initiatives and adhere to established policies and procedures.

Competencies Required:

  • Strong analytical and problem-solving abilities.
  • Effective communication skills, including listening and writing.
  • Attention to detail and proficiency in MS Office and EHR systems.
  • Ability to work independently and manage stress effectively.

Patient Demographics: Adult and geriatric populations.

Work Environment: Clinical setting with potential exposure to bloodborne pathogens; personal protective equipment (PPE) is mandatory.

Physical Requirements: This role involves alternating between sitting and standing, occasional walking, and lifting/carrying weights up to 75 lbs.

Qualifications:

  • Graduate of an accredited nursing program.
  • Preferred: One year of nursing experience.
  • Licenses: Current RN licensure in Iowa, BLS or CPR certification, and mandatory reporter training for child and dependent adult abuse.
