Job Overview:

The Nurse Practitioner utilizes administrative assistance while providing patient care services through digital platforms. The workload is contingent upon patient needs.

You will collaborate with a committed team of professionals to independently deliver comprehensive, evidence-based care, which includes precise assessment, diagnosis, treatment, management of health issues, health counseling, and planning for patient disposition.

Encounters are recorded using an electronic health record (EHR) system. This position reports directly to the Virtual Care SPM.

Key Responsibilities:


  • Patient-Centered Quality and Safety
  • Assess primary care, acute, chronic, and transitional care patients, while also providing health education and counseling, and planning for patient disposition for individuals aged 18 months and older
  • Offer patient counseling, including pregnancy prevention, STI prevention/safer sex practices, contraceptive care guidance, and medication management
  • Instruct patients on health maintenance and address patient care inquiries
  • Document all patient interactions within an EHR in accordance with established policies and procedures
  • Coordinate care for our patients with both internal and external colleagues, including the broader patient-centered medical home, ensuring the highest standards of care are maintained at all times
  • Effectively collaborate within a patient care team, which includes fellow Providers, Collaborative Physicians, para-professionals, Pharmacists, and other healthcare team members

  • Continuously reprioritize throughout the day to meet patient and business needs
  • Support the overall patient experience by effectively managing both clinical and non-clinical responsibilities as well as patient expectations
  • Quickly adapt to new service offerings and patient care models
  • Assist with the recruitment, development, and evaluation of Practice staff
  • Complete necessary tasks for clinic operations, including but not limited to inventory management, following up on lab results, answering incoming calls, ordering supplies, and maintaining clinic cleanliness

  • Remain accountable for managing business needs, including but not limited to budget, payroll, inventory, insurance billing, and payment collection
  • Take ownership of your practice's success through the implementation of the clinic-level business plan designed to achieve identified business objectives
  • Ensure complete and accurate payment for services through thorough documentation in the patient chart, verification of payment methods (insurance, cash, or a combination) for each visit, and collection of payments as directed by the EMR
  • Complete revenue cycle management tasks, including collecting payments at the time of service for all visits and preparing nightly deposits.

  • Actively engage in professional development through professional groups, committees within the organization, and/or additional external experiences
  • Maintain self-awareness and professionalism regarding individual actions and their impact on the clinic, practice, and healthcare industry
  • Continuing education, including what is required to maintain employment, is the responsibility of the provider

Experience:

  • Minimum of two years of relevant medical experience
  • Basic Life Support (BLS) certification
  • A minimum of high-speed/broadband internet connectivity with a download speed of at least 25 Mbps and an upload speed of 3 Mbps. While a speed test is not required on the resume, candidates should be aware of this requirement.
  • Must possess an unrestricted license in two of the states listed and have the ability to obtain multi-state/compact privileges and licensure in non-compact states as required by the business.

Education:

Completion of a Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role is required.


Candidates must be licensed in at least three of the following states: DC, DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, VT
