St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is dedicated to providing comprehensive care for individuals aged 55 and older who are managing chronic illnesses or disabilities. Our innovative approach ensures that participants receive coordinated medical care, allowing them to maintain their independence within their homes and communities.
This is a full-time role within our Day Center, with standard hours from Monday to Friday, 8:00 am to 4:30 pm.
Key Responsibilities:
Under the guidance of the Day Center Supervisor, you will be responsible for:
- Assisting participants with daily living activities and providing restorative and supportive care.
- Helping to organize and implement engaging activity programs.
- Monitoring the health status of participants and ensuring a safe environment.
Qualifications:
- Education: High school diploma or equivalent. Current certification as a Certified Nurse Assistant in California is mandatory.
- Experience: At least one year of experience working with the elderly or frail population is required. Relevant training or experience in the assigned role is essential.
- Certification: Current CPR (BLS) and First Aid certification from the American Heart Association is necessary.
