Job Summary

LHC Group is seeking a skilled Physical Therapist to join our team in Los Lunas, New Mexico. As a Physical Therapist with our company, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Evaluate patients' functional status and physical therapy needs, developing and implementing individualized treatment plans to improve their overall well-being.
  • Provide physical therapy services within the scope of practice, in accordance with the plan of care and in coordination with other healthcare team members.
  • Monitor patients' progress, reporting any changes or concerns to the nurse supervisor and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using evidence-based techniques.
  • Communicate with patients, families, and healthcare team members regarding the plan of care, functional limitations, and progress toward goals.
  • Visit patients in their homes, providing services as needed and in accordance with the plan of care.
  • Document patient progress and outcomes, submitting progress notes and other required documentation in a timely manner.
  • Participate in staff conferences and committees as requested by the agency.
  • May supervise home health aides and physical therapy assistants, as per LHC Group policy and state regulation.
  • Participate in ongoing education and professional development to maintain and enhance clinical knowledge and skills.
Requirements
  • Current Physical Therapy licensure in the state of New Mexico.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
