Primary Care Nurse Practitioner- Berwick, PA

We are seeking a skilled and dedicated Advanced Practice Professional to join our growing team of medical experts in Berwick, PA. As a Nurse Practitioner, you will play a critical role in providing direct patient care, discussing symptoms and concerns, and performing thorough examinations to diagnose medical problems and prescribe medication or create treatment plans when necessary.
You will also administer treatment, observe patients' functioning over time, record medical histories, perform physical exams, administer diagnostic tests, and create patient care plans. In collaboration with our physicians, you will treat advanced illnesses or injuries as required.
To be successful in this role, you should possess empathy but resolute decision-making skills, attention to detail, cultural competence, and exceptional teamwork abilities.
Duties and Responsibilities:
- Perform physical examinations and screenings of patients.
- Record patient medical histories and symptoms.
- Facilitate new patient consultations and health history reviews.
- Create patient care plans and contribute to existing ones.
- Order, administer, and analyze diagnostic tests, including bloodwork.
- Review and interpret test results to diagnose health problems.
- Recommend further treatment options or future appointments if needed.
- Monitor and operate medical equipment.
- Administer medicine and other treatments.
- Provide referrals to other practitioners when necessary.
- Detect changes in the patient's health and modify treatment plans, as needed.
- Offer after-care/ongoing care information and resources to patients and their families.
- Consult with healthcare professionals and families.
- Train patients and their families to manage and prevent illness and injury.
- Maintain accurate patient records, including test results, immunizations, condition changes, etc.

Requirements:
- New Grads considered for PCP; specialties require at least 6 months' experience
- Active NP license in IN
- DEA license or willingness to obtain
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
- Ability to work effectively as part of a team, alongside physicians and other nurses.
- Proficiency in EHR medical charting system and computer-based data management programs and information systems.
- Solid understanding of relevant federal and state regulations, including HIPAA and OSHA.
- Exceptional verbal and written communication skills.
- Knowledge of disease prevention.
