Job SummaryWe are seeking a highly skilled Bilingual Field Nurse Case Manager to join our team in Phoenix, AZ and surrounding areas. As a Field Case Manager, you will work remotely from home, traveling locally to doctors' offices, hospitals, and other locations to provide medical case management services for work-related injuries.
Key Responsibilities- Assess, plan, coordinate, implement, and evaluate the medical case management of injured/disabled individuals
- Work with insurance carriers, medical care providers, attorneys, employers, and employees to monitor the progress of injured workers and report results back to the employer and insurance carriers
- Develop and implement individualized care plans to ensure optimal outcomes for patients
- Collaborate with multidisciplinary teams to provide comprehensive care and support
- Provide education and support to patients and their families to promote self-management and independence
- Current, unencumbered Registered Nurse (RN) license in Arizona
- Experience in case management of workers' compensation cases, preferred
- Recommended certifications (CCM, CRRN, CDMS, CRC) preferred
- Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
- Ability to operate autonomously with minimal oversight
- Skilled at patient education
- Valid driver's license, and good driving history
- Bilingual (Spanish)
