Therapeutic Mentor Job Description

Overview:

We are seeking a compassionate and driven Therapeutic Mentor to join our team at Advocates. As a Therapeutic Mentor, you will work with children and adolescents up to age 21 through community-based activities, providing clinical and supportive services to help them manage various challenges.

Responsibilities:

  • Engage youth in social, recreational, athletic, and artistic activities in the community.
  • Participate in provider team meetings and collaborate with youth, family, and collaterals to support healthy functioning in the community.
  • Implement interventions to support youth and parent/guardian/caregiver.
  • Provide member transportation as needed for service delivery.
  • Offer coaching in decision-making situations.
  • Attend necessary meetings, such as care planning, school, and hospital discharge meetings.
  • Provide referrals and linkage to appropriate supports and services.
  • Conduct member outreach to engage members and families.

Qualifications:

  • Bachelor's degree from an accredited university and one year of experience working with children/adolescents/transition age youth OR Associate's degree from an accredited school and one year of experience working with the target population OR High school diploma or GED and a minimum of two years of experience working with children/adolescents/transition age youth.
  • 21 years of age or older.
  • Working knowledge of CBHI system of care.
  • Outstanding customer service skills.
  • Exceptional interpersonal skills to facilitate working relationships with referral sources, colleagues, and partner agencies.
  • Strongly prefer a demonstrated understanding of and competence in serving culturally diverse populations.
  • Basic computer skills.
  • Ability to communicate effectively verbally and in writing.
  • Valid driver's license and access to a vehicle.
