Job Title: Home Infusion Nurse

InfuCare Rx Inc, a national Specialty Infusion Company, is seeking a skilled Home Infusion Nurse to join our patient-centric pharmacy team. As a leader in the Specialty and Home Infusion space, we offer a dynamic and supportive work environment that fosters growth and development.

Job Summary:

The Home Infusion Nurse will be responsible for providing high-quality patient care and education to patients receiving infusion therapy in the comfort of their own homes. This role requires excellent nursing skills, attention to detail, and strong communication skills to work collaboratively with patients, families, and healthcare teams.

Responsibilities:
  • Administer infusion therapy to patients in their homes, ensuring accurate dosing and monitoring of patient responses.
  • Conduct thorough patient assessments, identify potential complications, and take appropriate action to prevent or manage them.
  • Provide education and support to patients and families on infusion therapy, medication management, and disease management.
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
  • Maintain accurate and timely documentation of patient care, including medication administration records and progress notes.
Requirements:
  • Current and unrestricted RN license.
  • 3 years' experience as an RN, preferably with some home care experience.
  • A Bachelor of Science in nursing or an associate degree in nursing from an accredited and approved nursing program.
  • Current CPR certification.
  • Proof of eligibility to work in the US.
  • Successfully pass annual competency checks.
  • Clean background and criminal record and must pass drug test.
Physical Demands:

Required to stand, walk, sit, talk, and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
