Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team at HealthPro Heritage. As an Occupational Therapist, you will play a vital role in helping our residents achieve their maximum potential in daily living skills.

1. Evaluate and treat residents in accordance with physician orders, utilizing therapeutic procedures and assistive devices to maximize functional potentials.
2. Participate as a member of the interdisciplinary team, providing support and information within the area of Occupational Therapy.
3. Review resident information and evaluate its impact on the assessment and treatment process.
4. Conduct and document thorough functional assessments of residents, utilizing standardized procedures.
5. Develop and implement individualized treatment plans based on resident needs and containing achievable functional goals.
6. Comply with evaluation, treatment, and documentation guidelines, maintaining timely and accurate departmental records.
7. Enter progress notes in resident records following each treatment session.
8. Revise treatment plans as needed throughout the treatment process.
9. Document the course of treatment in a clear, concise discharge summary.
10. Orient residents to program services and treatment procedures, informing them of any potential risks.
11. Work with nursing staff, residents, and families to maximize resident use of functional abilities.

1. Bachelor/Masters of Science degree in Occupational Therapy.
2. Current license/certification in the state of practice.
3. Prefer supervision, training, and/or experience in a setting serving the same age/type of resident population.
4. Certification, if applicable, and demonstrated competency in identified areas of specialization.
5. Excellent oral and written communication and interpersonal skills.
6. Demonstrated ability to assess resident needs and develop and implement a comprehensive plan of care.
