Job Overview

CareConnectMD Inc is seeking a highly skilled Geriatric Care Coordinator Nurse Practitioner to join our team. This rewarding role offers the opportunity to work in a collaborative environment, providing comprehensive care to frail and elderly patients in long-term care settings.

Key Responsibilities
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients to identify their needs for services and support, including DME and supplies, acute medical services, behavioral health services, HCBS, and primary or specialty care.
  • Collaboration and Communication: Work closely with patients, families/caregivers, and healthcare providers to develop and implement personalized care plans that address each patient's unique needs and goals.
  • Care Coordination and Management: Manage a complex caseload of patients, ensuring timely completion of medical records, discharge summaries, and annual wellness visits, while maintaining accurate EMR charting and communicating assessment outcomes with patients and PCPs.
  • Medication Management and Review: Order medications (including IVs), labs, and other diagnostic testing, as well as review medication lists and adjust as necessary to ensure optimal patient outcomes.
  • Advocacy and Support: Serve as a patient advocate, facilitating communication with care teams and resolving issues that may impact patient care, while also participating in IDTs and peer review meetings.
Requirements
  • Education and Certification: Master's degree with an emphasis in adult/geriatric nursing program; Board certification as an adult/geriatric nurse practitioner; DEA registration; current CPR certification; state certification as an adult/geriatric nurse practitioner.
  • Experience: Preferably 1-2 years of experience in geriatrics and skilled nursing facilities, but not required.
  • Skills and Abilities: Strong background in geriatric and/or internal medicine; excellent communication and interpersonal skills; ability to work independently and manage multiple priorities; creative problem-solving skills; resourcefulness and adaptability in fast-paced environments.
Compensation and Benefits

We offer a competitive salary range of $ 120000 to $ $160000 per annum, commensurate with experience, depending on location, and benefits including health insurance, retirement savings plan, paid time off, and opportunities for professional growth and development.
